A Samaritan Family Health Clinic employee has been arrested after she allegedly stole and forged a prescription for a controlled substance.
On Thursday, June 7th, the New York State Police, assisted by the New York State Bureau of Narcotics Enforcement, arrested Heather L. Carr, 28, of Mullin Street in Watertown, and charged her with Criminal Possession of a Controlled Substance 4th degree (Class C Felony), Forgery 2nd degree (Class D Felony), Falsifying Business Records 1st degree (Class E Felony), and Obtaining a Controlled Substance by Fraud and Deceit (Unclassified misdemeanor).
Carr allegedly stole and forged a prescription for Vicodin/Hydrocodone, a controlled substance, from Dr. Ryan E. Tyler while employed at Samaritan Family Health Clinic, located at 1575 Washington Street.
After arraignment Carr was released pending Grand Jury action.
